$1950 annual investment for founding members

$200 deposit due at time of enrollment, remainder to be paid at or before your first visit.

This level of service provides a full suite of gynecology and women’s health services for women, and is designed to encompass a full spectrum of needs ranging from annual gynecology exam to weight loss management to preventative and sick care.

Services include the following:

  • Unlimited acute and non-acute office visits (both well visits and sick visits).

  • Comprehensive annual physical exam, breast and pelvic exam, pap smear and advanced wellness screenings

  • perimenopause, menopause, sexual health, weight management, hair loss, and hormone optimization

  • Quarterly follow-up

  • Prescription management

  • Specimen collection for labs (testing of specimen by a third-party laboratory is an Excluded Service)

  • Telehealth visits available when appropriate (for women in Alabama)

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Priority scheduling, longer visit times, minimal wait time, same-day or next-day appointments when available

  • Full care coordination and referrals to specialists when needed

  • Daughters of Thrive members may be seen ages 12-22 for $50/visit

Bloom Care Plan

Where wisdom meets renewal.

$1,200 investment

$200 deposit due at time of enrollment, remainder to be paid at or before your first visit.

Support for perimenopause and menopause. Guidance through hormonal shifts with expert evidence-based care. Sleep disturbances, vasomotor symptoms, and sexual health will also be discussed. The Bloom membership is ideal for women who currently have a relationship with a GYN but would like more personalized support navigating the hormonal changes of perimenopause and menopause.

Services include the following:

  • Initial consultation and review of pre-visit lab work

  • Prescription management. Filled prescriptions and/or purchased medications are Excluded Services.

  • Specimen collection for labs for in office visits (testing of specimen by a third-party laboratory is an Excluded Service)

  • Up to 4 follow-up visits over one year (generally recommended at 6 weeks and again at 6 months, as well as when needed for hormone optimization)

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Telehealth visits available when appropriate (for women in Alabama)

    Bloom does not include a yearly physical exam.

    Weight management may be discussed in the Bloom care plan but due to prescribing guidelines, weight management prescriptions are not included in the Bloom care plan.

Branch Care Plan

The Branch experience offers 6 visits to focus on thoughtful, physician-guided weight management. Through lab analysis, body composition review, and evidence-based recommendations, we help you pursue weight-loss goals with clarity and confidence. Monthly follow-ups, prescription guidance, and optional integration with wearable fitness trackers make this a modern, connected approach to long-term wellness

Weight management services include the following:

  • Initial consultation and review of pre-visit lab work

  • Recommendations, goals, and prescriptions for weight loss. Filled prescriptions and/or purchased medications are Excluded Services.

  • Specimen collection for laboratory collection (testing of specimen by a third-party laboratory is an Excluded Service)

  • Monthly follow-up

  • Monitoring of fitness wearables (Apple Watch, Oura Ring). Body composition analysis for in-person visits.

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Telehealth visits available when appropriate (for women in Alabama)

Branch does not include a yearly physical exam.

Seed Care Plan

Foundations in Bloom.  

$495 12-month investment

$200 deposit due at time of enrollment, remainder to be paid at or before your first visit.

Seed is designed for young women (ages 12–22) navigating the early stages of reproductive health. This offering includes first gynecology visits, cycle education, contraception counseling, and STD screening when indicated—all in a welcoming, age-appropriate setting.

 Services include the following:

  • First gynecology visit / yearly gynecology visit

  • Contraception counseling

  • Cycle education

  • STD screening when indicated

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Referrals to specialists when needed

    Most young women do not need a pelvic exam until age 21, and we will discuss indications or need for exam.

  • Flourish does not participate with insurance. Insurance will not be billed for any Program Services provided to the Member by Flourish. Flourish is not a participating provider with any third-party payor and will not submit claims or seek reimbursement or payment for Services from any third-party payor. The Member (or if the Member is a minor, the Member’s parent(s) or legal guardian(s)) will be solely responsible for payment for all Services rendered.

  • Any woman seeking a more personal, thoughtful approach can become at patient at Flourish. Already have an OB/GYN? You can still see Dr. Gooding for hormone or health optimization.

    Members’ daughters (ages 12–22) are welcome for a small per-visit fee.

  • Item description

  • Flourish and its staff have opted out of Medicare. Both the Member (or if the Member is a minor, the Member’s parent(s) or legal guardian(s)) and Flourish shall be prohibited by law from seeking reimbursement from Medicare for any Services provided.

    All Members who are or who become Medicare beneficiaries are required to sign the Opt-Out Private Contract (Medicare Patients).

  • After receiving your initial intake request, our care coordinator will call you to confirm scheduling and order your blood work prior to your appointment. You will also receive an invitation to the patient portal for self-scheduling.

  • Solo practice: Your care is always provided by Dr. Gooding

    Daytime Availability: Dr. Gooding responds to messages and requests during regular daytime business hours within a reasonable timeframe.

    After Hours: Dr. Gooding is not available after business hours or overnight. E-mails, voicemails and phone calls received outside business hours will be returned during business hours within a reasonable timeframe.

    Urgent needs: For urgent or emergency situations, please dial 911 or proceed to an emergency facility.

    Proactive approach: Most needs can be anticipated and managed during daytime business hours with same-day or next-day appointments when available.

    Goal: This model ensures you have Dr. Gooding’s full attention and energy during each interaction. You do not have to worry that your doctor is sleep-deprived after a long call night.

     

  • At this time, the at-home diagnostic kit is available exclusively to Thrive members. It includes flu and covid test, a pH test, urinalysis supplies, along with other tools to support your telehealth visit. Step-by-step instructions are included so you can get answers quickly without an extra office trip.

  • Dr. Gooding is retiring her obstetric practice and will no longer be delivering babies.

  • Flourish does not participate with insurance but will provide a superbill you can submit. Many members also use FSA or HSA funds—check with your plan to confirm.

  • Any woman seeking a more personal, thoughtful approach can become at patient at Flourish. Already have an OB/GYN? You can still see Dr. Gooding for hormone or health optimization.
