$1950 annual investment

Payment is due at time of enrollment. Option to pay in one twelve-month installment or two six-month installments.

This level of service provides a full suite of gynecology and women’s health services for women, and is designed to encompass a full spectrum of needs ranging from annual gynecology exam to weight loss management to preventative and sick care.

Services include the following:

  • Unlimited acute and non-acute office visits (both well visits and sick visits).

  • Comprehensive annual physical exam, breast and pelvic exam, pap smear and advanced wellness screenings

  • perimenopause, menopause, sexual health, weight management, hair loss, and hormone optimization

  • Quarterly follow-up

  • Prescription management

  • Specimen collection for labs (testing of specimen by a third-party laboratory is an Excluded Service)

  • Telehealth visits available when appropriate

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Priority scheduling, longer visit times, minimal wait time, same-day or next-day appointments when available

  • Full care coordination and referrals to specialists when needed

  • Daughters of Thrive members may be seen for gynecology concerns ages 12-22 for $50/encounter (sick visits not included for daughters as Dr. Gooding is not a pediatrician)

Seed Membership

Foundations in Bloom.  

$495 12-month investment

Seed is designed for young women (ages 12–22) navigating the early stages of reproductive health. This offering includes first gynecology visits, cycle education, contraception counseling, and STD screening when indicated—all in a welcoming, age-appropriate setting.

 Services include the following:

  • First gynecology visit / yearly gynecology visit

  • Contraception counseling

  • Cycle education

  • STD screening when indicated

  • Direct physician access via our messaging app and email during business hours for non-urgent matters

  • Referrals to specialists when needed. Dr. Gooding is not a pediatrician. Seed membership does not include sick visits.

    Most young women do not need a pelvic exam until age 21, and we will discuss indications or need for exam.

  • Flourish does not participate with insurance. Insurance will not be billed for any Program Services provided to the Member by Flourish. Flourish is not a participating provider with any third-party payor and will not submit claims or seek reimbursement or payment for Services from any third-party payor. The Member (or if the Member is a minor, the Member’s parent(s) or legal guardian(s)) will be solely responsible for payment for all Services rendered.

  • Any woman seeking a more personal, thoughtful approach can become at patient at Flourish. Already have an OB/GYN? You can still see Dr. Gooding for hormone or health optimization.

    Members’ daughters (ages 12–22) are welcome for a small per-visit fee.

  • To enroll, click the enrollment link at the top of this page, complete your forms, and you’ll get access to my members-only contact info and online scheduling. Your first visit can be in-person or virtual.

  • Item description

  • Flourish and its staff have opted out of Medicare. Both the Member (or if the Member is a minor, the Member’s parent(s) or legal guardian(s)) and Flourish shall be prohibited by law from seeking reimbursement from Medicare for any Services provided.

    All Members who are or who become Medicare beneficiaries are required to sign the Opt-Out Private Contract (Medicare Patients).

  • Dr. Gooding offers both in-person and virtual visits to address your specific health needs.

  • After receiving your initial intake request, our care coordinator will call you to confirm scheduling and order your blood work prior to your appointment. You will also receive an invitation to the patient portal for self-scheduling.

  • Solo practice: Your care is always provided by Dr. Gooding

    Daytime Availability: Dr. Gooding responds to messages and requests during regular daytime business hours within a reasonable timeframe.

    After Hours: Dr. Gooding is not available after business hours or overnight. E-mails, voicemails and phone calls received outside business hours will be returned during business hours within a reasonable timeframe.

    Urgent needs: For urgent or emergency situations, please dial 911 or proceed to an emergency facility.

    Proactive approach: Most needs can be anticipated and managed during daytime business hours with same-day or next-day appointments when available.

    Goal: This model ensures you have Dr. Gooding’s full attention and energy during each interaction. You do not have to worry that your doctor is sleep-deprived after a long call night.

     

  • At this time, the at-home diagnostic kit is available exclusively to Thrive members. It includes flu and covid test, a pH test, urinalysis supplies, along with other tools to support your telehealth visit. Step-by-step instructions are included so you can get answers quickly without an extra office trip.

  • Dr. Gooding is retiring her obstetric practice and will no longer be delivering babies.

  • Flourish does not participate with insurance but will provide a superbill you can submit. Many members also use FSA or HSA funds—check with your plan to confirm.
